Larger DSAEK grafts linked to reduced graft failure

The American Academy of Ophthalmologists reported that investigators analyzed the influence of graft size on outcomes in patients who had Descemet stripping automated endothelial keratoplasty (DSAEK) for Fuchs endothelial dystrophy or pseudophakic bullous keratopathy. Among 131 patients, 8.40% experienced endothelial graft failure: 17.86% with trephine sized grafts <9 mm; 7.69% for 9 mm and 3.84% for 9.5 mm. Graft failure was significantly associated with endothelial cell density (P=0.039) and graft trephine size (P=0.04), suggesting that larger grafts make it possible to transplant a greater number of cells, potentially increasing long-term graft survival.
